Our Vision

The Halton Geriatric Mental Health Outreach Program (HGMHOP), a satellite program of St. Joseph's Healthcare Hamilton Regional Geriatric Psychiatry Service, is a unique and innovative program dedicated to seniors with late onset mental health problems, addictions and/or behavioural disturbances and their caregivers. We serve adults 65 years of age and over residing in the region of Halton and Northwest Mississauga living in their own homes, retirement or supportive housing and Long Term Care Homes. The age range is flexible for adults with early onset dementia.

We provide clinical outreach services to those seniors who cannot or will not seek traditional service e.g. going to an office.

The service delivery catchment of the Halton Geriatric Mental Health Outreach Program is defined as:

  • Region of Halton, i.e. Burlington, Oakville, Milton, Acton and Georgetown
  • Northwest Mississauga covering the area north of the QEW, west of Mavis Road and south of the 401 or as determined by postal codes: L5K, L5L, L5M, L5N, L5C, L5V, L5W

In the Mississauga Halton LHIN, St. Joseph's Healthcare Hamilton and Trillium Health Centre share in providing geriatric mental health outreach services. Although each program has a defined geographic catchment area we are flexible and work together to best match the resources and meet the needs of the senior and their family/caregivers.
